Diagnosica makes one of the products compared on this page, so you should weigh what follows accordingly. Every price and feature here was checked on the vendors' own sites on 14 August 2026, and where a competitor is better the page says so.

What an AI clinical case simulator actually does

An AI clinical case simulator is software where you take a history from an artificial patient, order investigations, commit to a differential and a likely diagnosis, outline a management plan, then receive structured scoring and feedback. Some products include all steps, others limit you to history, ordering, or reasoning.

The core loop mirrors clinic or call, you ask open and directed questions, request labs and imaging, commit to a diagnosis, write or defend a plan, and get debriefed against a rubric. Speech or text intake varies, as do scoring depth and debrief clarity.

Search the term and the first page is all vendors, with no independent side by side, as of August 2026. The players we compare here are Diagnosica, Neural Consult, Full Code Medical, MedSimAI, MediSim AI, plus a free iPhone app by a single developer.

A case simulator is one tool on your bench, beside spaced recall, question banks, and anatomy or pharm aids. For a sweep of options, see AI study tools compared.

The five products, side by side

Here are five case simulators pulled into one view, with claims and prices taken from vendor pages as of August 2026. Two have almost the same name but are different products, MedSimAI at MedSimAI, sold to schools and MediSim AI at MediSim AI's case simulator.

Among consumer subscriptions that publish a price, Neural Consult's case simulator is cheaper as of August 2026, at $24.99 a month and $224.99 a year, than Diagnosica's $29 a month and $249 a year. Those figures are on Neural Consult's published prices.

  • Diagnosica Patient modality: voice or text · Cases claimed: 74 cases across 14 specialties, 3 difficulty bands · Published price: Free tier of one case a week; $29/mo or $249/yr; UK cards £22/mo or £189/yr · Who it is sold to: individuals
  • Neural Consult Patient modality: voice or text · Cases claimed: not published as a number; the page says hundreds of cases across dozens of degrees, plus unlimited cases you create yourself · Published price: free starter tier; $24.99/mo; $224.99/yr · Who it is sold to: individuals
  • Full Code Medical Patient modality: AI patient conversations · Cases claimed: stated inconsistently on its own site, 250+ in some places and 200+ in others, across 31 specialties · Published price: free tier with 4 cases; $11.99/mo or $71.99/yr; $99.99/yr with CME · Who it is sold to: individuals and institutions
  • MedSimAI Patient modality: voice or chat · Cases claimed: 71 clinical scenarios · Published price: not published · Who it is sold to: institutions: medical schools, residency programs, dentistry, physical therapy and veterinary programs
  • MediSim AI Patient modality: not stated · Cases claimed: not stated · Published price: not published · Who it is sold to: individuals

Two of the five publish no price at all, MedSimAI and MediSim AI, which usually means they’re sold to a school rather than to you.

Full Code contradicts itself on its own case count as of August 2026, so treat that number as approximate, see Full Code's own product page for the inconsistent wording.

What voice practice actually gets you

Talking out loud to a simulated patient exercises timing, phrasing, and composure in a way typing can't. You commit to a question without editing, sit with silence, manage interruptions, and correct yourself in real time, that's much closer to an exam bay or ward round than a chat window.

Diagnosica offers voice or text as of August 2026.

Neural Consult says its simulator is voice enabled and patients talk back, MedSimAI runs voice or chat encounters, and Full Code describes AI patient conversations, all as of August 2026, while MediSim AI says nothing about voice.

That matters because the performance piece is spoken, whether you're cold calling a differential on rounds or working through a station where a patient narrative unfolds. Typing lets you smooth edges before you hit send, speaking doesn't.

None of this rehearses physical examination. Diagnosica has no examination content, signs are given in the history when relevant and you reason from them, you can't elicit them.

If you want a station shaped drill for the spoken piece, see a write up on simulators built for OSCE stations. If you want to test that feel tonight without booking, Start a free case tonight.

What one case actually contains

One open case tells you more than a landing page, so here is what you see inside Diagnosica's Surgery Easy case Belly pain and fever, 24M.

Titled "Belly pain and fever, 24M", tagged Surgery, set to Easy, and every case in the library is written and signed off by a doctor.

The patient is Cameron Hayes, 24, previously well, uncomfortable and guarded, lying still, answering clearly but wincing with movement.

His ideas, concerns, and expectations are prewritten, from first thinking food poisoning to worrying about worsening right-sided pain and fever, wanting pain sorted and to know if surgery is needed.

Vitals are HR 98, RR 18, BP 124/76, SpO2 99 percent, temperature 37.8 C, and he opens with, "I've had belly pain since yesterday and I feel feverish."

There are 16 history facts, two volunteered, four on ordinary questions, ten only with specific probes, including migrating pain volunteered and appetite loss mentioned only if asked.

Two items are tagged as required red flags, diffuse earlier pain across the abdomen, and steadily worsening pain with fever over the last several hours.

Seventeen investigations each carry a reason, such as a urine dipstick to exclude UTI or renal colic, and the differential lists seven alternatives including testicular torsion and a ureteric stone.

Conditional scripting means it feels like a consult, and you see exactly why each question, test, and differential slot is there.

Labeled case: 16 history facts split into volunteered, question, probe; 2 red flags; 17 tests with notes; 7 differential

Where each one breaks

Diagnosica. There are 74 cases, small next to Full Code's number, and the library is lopsided. Seventy three of 74 patients are male, the single female case is obstetric, with no psychiatry, no family medicine, one dermatology case, and two pediatric cases. There is no physical examination content, it costs more than Neural Consult month for month and year for year, and the product is live and early so rough edges are expected.

Neural Consult. It publishes no case count as of August 2026, so a buyer cannot tell how much of the library is authored versus generated on request from its custom case feature.

Full Code. It contradicts its own case count across its site as of August 2026, saying 250+ in one place and 200+ in another, which leaves you guessing what you are planning against.

MedSimAI and MediSim AI. As of August 2026, neither publishes a price, and MediSim AI does not state a case count or whether you can speak to the patient.

The app store tier. A free iPhone app called "AI Medical Case Simulation," from Pera Soft Dijital Cozumler Anonim Sirketi, version 1.1.5, carried 24 US App Store ratings on 14 August 2026 on its App Store listing. Nothing on the listing says who wrote its cases or whether a doctor checked them, and for contrast Full Code's listing carried 6,295 ratings on the same store the same day.

When to pick one of the others

Pick Full Code, a trading business, if you want the biggest library and the lowest consumer price, as of August 2026. See Full Code's individual plans for the $11.99 per month or $71.99 per year individual plan, a free tier with 4 cases, and a $99.99 per year CME option, and check Full Code's App Store ratings for 6,295 US ratings averaging 4.77, all as of August 2026.

Pick Neural Consult if you want one subscription that covers flashcards, board questions and case simulation together, not a single purpose case simulator. Its $24.99 per month and $224.99 per year plans, as of August 2026, will matter if you care about price and bundle value.

Talk to MedSimAI if you are buying for a school or a residency program, because it's built around curriculum integration. It does not publish a consumer price.

If you have no budget, start with simulators that cost nothing, then layer paid tools later.

How to choose in ten minutes

  1. Open the free tier before you pay.
  2. Ask who wrote the cases and whether a doctor signed them off, because Diagnosica's are and most listings don't say.
  3. Count the cases you would actually use, since a library weighted toward one specialty shrinks the headline number.
  4. Check whether you can speak to the patient if that's your weak spot.
  5. Check what happens after the case ends, since scoring and a debrief turn a session into feedback, and compare with the Step 3 case format.
If you're deciding on an AI clinical case simulator, Diagnosica has a free tier of one case a week. There's a no signup demo on the landing page that takes about 3 minutes and marks you at the end, so Run a case free.
