Practise history taking with an AI patient
Updated 4 November 2026
Diagnosica gives you a patient on video to take a history from, by voice or text. The patient answers in their own words and only tells you what you ask about, so a question you skip stays skipped. Then you order tests, commit to a diagnosis and plan, and get a scorecard out of 100 in which history taking is one of four areas.
It's free to start, with no card, on the web, iPhone and Android.
Diagnosica is an AI clinical simulator for medical students and junior doctors. You take a history from an AI patient on video, by voice or text, order labs and real imaging, commit to a diagnosis and management plan, and get a competency scorecard.
Why practise history taking on an AI patient?
Knowing the structure of a history is the easy part. The hard part is choosing the next question, picking the negatives that matter and knowing when you have enough to act. You get better at that by doing it, and an AI patient lets you do it at any hour, as often as you like, with the same marking every time.
What a case looks like
- Meet the patient. You get a presenting complaint and a patient on video. Speak or type your questions.
- Take the history. Start open, then go after the detail: onset, character, red flags, past history, medicines, family and social history. The patient won't bring up a symptom you never asked about.
- Act on it. Ask for examination findings, order labs and real imaging, then commit to a diagnosis, your differentials and a management plan.
- See what you missed. The scorecard marks history taking, investigations, diagnosis and management (25 points each), with teaching points on what you missed.
A full case takes around 15 to 20 minutes. There are 136 cases across 18 specialties, each written for teaching and clinically reviewed before it goes live.
| Area | Points |
|---|---|
| History taking | 25 |
| Investigations | 25 |
| Diagnosis | 25 |
| Management | 25 |
| Total | 100 |
Can I speak instead of typing?
Both work. Speaking is closer to a real consultation; typing suits a library or a bus. A free account includes 30 minutes of voice a month, and you can type once that runs out.
Learn the method, then practise it
Our guides cover the method: how to take a focused history, choosing pertinent negatives and how to clerk a patient. Read one, then try it on a case.
What it doesn't do
- Hands-on examination. You ask for findings rather than examining anyone yourself, so physical skills stay with real patients and tutors.
- One exam's mark scheme. It isn't affiliated with any exam board and doesn't predict a mark.
Common questions
Can I practise history taking online for free?
Yes. Diagnosica is free to start with no card. A free account gets 5 cases in your first week, then 3 cases a week, plus unlimited 2-minute quick cases, with every case in the library open.
Do I get feedback on my history?
Yes. Every case ends with a scorecard out of 100 across history taking, investigations, diagnosis and management (25 points each), with teaching points on what you missed. It's formative feedback, not an exam mark.
Does it help with OSCE, UKMLA or PLAB history stations?
It practises the part of those stations that is a conversation with a patient: what to ask, in what order and what to do with the answers. It doesn't mark examination technique or follow any one exam board's mark scheme.
How much does Diagnosica cost?
Diagnosica is free to start, with no card. A free account gets 5 cases in your first week, then 3 cases a week, plus unlimited 2-minute quick cases, with every case in the library open. Does Diagnosica work on Android and iPhone?
Yes. Diagnosica has native apps on Google Play and the App Store, and runs in any browser at app.diagnosica.com. It is one account across all three, so your cases, streak and league position follow you from a laptop to your phone and back.
Is Diagnosica legit, and who makes it?
Diagnosica is a small, independent company operated by CC Global Group LLC, live on the web, the App Store and Google Play. Cases are written for teaching and clinically reviewed before they go live. It is a learning tool, not a medical device, and it is not affiliated with any exam board.
